Caring for someone with dementia is one of the most demanding responsibilities a family can take on. As memory loss progresses, caregivers often find themselves managing medications, preventing safety risks, coordinating appointments, and providing around-the-clock support, all while trying to balance work, family, and their own well-being.

Recognizing these challenges, Medicare introduced the GUIDE Program to give families better access to education, care coordination, and respite services that make dementia caregiving more manageable.

What is the GUIDE Program?

The GUIDE Program (Guiding an Improved Dementia Experience) is a Medicare initiative that brings together medical providers, care coordination, caregiver education, and respite services to improve support for people living with dementia and their caregivers.
Through participating healthcare organizations, eligible individuals receive coordinated dementia care while family caregivers gain education, ongoing guidance, and temporary respite care to help them continue providing care at home.

GUIDE Program FAQs

What does GUIDE stand for?

GUIDE stands for Guiding an Improved Dementia Experience. It’s a Medicare model focused on improving support and quality of life for people with dementia and their caregivers.

Who is eligible for the GUIDE Program?

Individuals with a diagnosed dementia or Alzheimer’s condition who are enrolled in Medicare may qualify. Families Choice Home Care can help you determine eligibility and guide you through next steps.

Does Medicare cover the GUIDE Program?

GUIDE is a Medicare initiative, and covered services are provided at no direct cost to eligible participants under the program. Our team can walk you through what’s included and answer specific questions about your situation.

Does Families Choice Home Care provide the GUIDE Program?

Families Choice Home Care helps eligible families access the Medicare GUIDE Program through our partnership with Loma Linda University Medical Center. Once enrolled, we provide compassionate in-home dementia care and respite services while helping families navigate available support.

What is respite care, and how does it help caregivers?

Respite care provides temporary, professional care for your loved one so that family caregivers can rest, recharge, or attend to other responsibilities. It’s one of the core supports offered through the GUIDE Program and helps prevent caregiver burnout.

What happens if families wait too long to get support?

It’s completely understandable to put this off; many families do. But caregiver burnout tends to build gradually, and it can become harder to carry alone over time. Reaching out earlier, even just to ask questions, can make a meaningful difference for both you and your loved one.
