Extended Alzhemer’s & Dementia Care Services

Our enhanced in-home dementia care services expand on our standard offerings to provide comprehensive support 24 hours a day.  With 24/7 in-home dementia or Alzheimer’s care, your loved one will have an experienced caregiver with them at all hours of the day—ensuring safety and comfort day and night.

This service can be used as respite care, allowing primary caregivers extended periods of rest from caregiving. Or, our incredible team of caregivers can be a mainstay of a loved one’s plan to age in place. By hiring a team of experienced caregivers to work around the clock, seniors with dementia or Alzheimer’s get to remain in the comfort of their own homes and communities, surrounded by the people and things they love most.

It’s important to highlight that “24-hour home care” is not the same as “live-in-care.” Rather than caregivers moving into the home or requiring a room of their own, 24-hour dementia home care services utilize a team of six to eight experienced caregivers who rotate shifts throughout the week. In most cases, shifts last between six to eight hours, after which another caregiver arrives for the next shift.

As with all of our non-medical senior home care services, 24-hour dementia and Alzheimer’s care is personalized to our client’s needs, continuously adapting to age or health-related changes as they arise.

Our In-home Alzheimer’s & Dementia Assessment Process

Alzheimer’s and dementia affect each person differently. Understanding the patient’s unique challenges is essential for creating a care plan addressing their situation. We utilize a proven three-step approach to evaluate cognitive, emotional, and physical well-being.

Step
1

Personalized Care Planning Meeting

Our team begins with a thorough evaluation that forms the basis of our personalized services. We sit down and start gathering information by talking to family members, doctors, etc. This enables us to tailor our care to each patient’s situation and preferences.

Our discussion covers key areas, including:

  • Understanding Daily Needs: We assess how the patient’s cognitive challenges impact their daily routines and behaviors.
  • Gathering Care Preferences: We collect information on preferences, routines, and any specific triggers that may cause stress or confusion.
  • Caregiver Matching: Caregivers are selected based on their ability to handle specific cognitive care needs and their compatibility with the patient’s personality.
  • Addressing Behavioral Challenges: We’ll discuss potential behavioral issues, such as confusion or agitation, and create strategies to manage them.
  • Encouraging Family Involvement: We promote open conversations with family members to ensure they feel supported and informed during the process.
  • Transparent Costs: We provide a detailed breakdown of service costs, tailored to the patient’s unique cognitive care requirements.

Step
2

Customized Care Plan Development

Our team takes their findings and creates a customized care plan. It covers areas that include:

  • Daily Schedule: We put together a daily schedule that aligns well with the client’s needs.
  • Ongoing Needs: Our caregivers provide mobility support when and where necessary. Nutritional guidance and emotional care are included to meet the client’s changing requirements.
  • Cognitive Stimulation: The care plan includes activities that engage mental functions, such as memory exercises or puzzles.
  • Social Interaction: Regular opportunities for social engagement are provided to promote emotional well-being.
  • Safety Measures: The plan includes adjustments to the home environment to prevent accidents and ensure safety.

Step
3

Collaborative Review & Implementation

We collaborate with patients, their families, and care professionals to make sure the plan is effective and continuously adjusted to meet changing needs. The process emphasizes ongoing support and flexibility to align with the evolving requirements of the individual.

We approach this through:

Ongoing Monitoring and Evaluation: We continuously monitor progress and evaluate the effectiveness of the plan. This enables us to identify areas for improvement, make adjustments as needed, and ensure the patient receives the best possible care.

Reassessing and Revising: The patient’s needs can shift over time. We regularly reassess and modify the plan to accommodate those adjustments. Our approach remains adaptable so that we can continue to provide consistent, personalized support.

Finalizing the Care Plan: We provide patients and their families with a clear understanding of the services and support they will receive. We also establish open lines of communication to address any questions or concerns that may arise.
